Semi-quantitative, paper-based detection of the inflammatory biomarkers, C-reactive protein and procalcitonin using laser-patterned multiplex technology

C- reactive protein (CRP) and procalcitonin (PCT) have been shown as useful biomarkers to improve triage speeds and prevent the inappropriate use of antibiotics for infection such as pneumonia. Here, we present a novel and exciting solution for rapid, semi-quantitative and multiplexed detection of CRP and PCT that may indicate an infection, and whether the causative pathogen is bacterial or viral to guide the administration of antibiotic treatment. Our novel approach uses an advanced lateral flow device (LFD) with multiple parallel flow-paths produced via the precise partitioning of the single flow-path of a standard LFD. Each flow path has a unique detection capability which permits tailored detection of the biomarkers within a cut-off range, with no interference or cross-reactivity between lanes. We tested this multiplexed LFD using these biomarkers spiked in human serum and successfully detected CRP, below 20 µg/ml, between 20 µg/ml and 100 µg/ml and above 100 µg/ml in line with the UK’s NICE guidelines for CRP stratification. Similarly, we detected PCT at concentration above 0.5 ng/ml. This multiplex near-patient assay has potential for development into a rapid triage and treatment of patients with suspected pneumonia.
