Extracellular vesicles (EVs) are attracting attention as potential biomarkers because they contain molecular cargo from their cell of origin that might reveal information about health and disease. However, EVs are hard to measure because they are small and heterogeneous. Flow cytometry, which can resolve heterogeneity in populations of cells, struggles to measures small, dim EVs. Recent developments in flow cytometry instrumentation and assay design enable us to quantitatively detect, size, and analyze the cargo of individual EVs and to catalogue their diversity. This approach allows EVs to be measured directly in diluted biofluids without the need for wash steps. I will report on the numbers and molecular diversity of EVs in plasma, CSF, and urine, and the variations observed in disease models and patient samples.
